MEDICARE SUPPLEMENT PLAN INFORMATION FOR 2012

The Drug Formulary lists all the drugs that are covered, if your drug is not listed on the formulary then ask your doctor if you can substitute for one on the list.  We cannot add drugs to the formulary in the middle of a policy year. 

You can order your prescriptions through the mail with a cheaper co-pay then at a local pharmacy but you can only get a 30 day supply with the  mail-in-service.  You can always obtain a 30 day supply at the local pharmacy.  Mail in or local order – No 90 day supplies are available.

If you are a diabetic, you should be ordering your supplies through a vendor like Liberty; their phone is 800-867-0689.  These vendors work with Medicare and your supplement so you will not have monies out-of-pocket.

Dental coverage through Careington allows you to use your own dentist but you can get a better discount if you use a dentist on the plan.  To see if your dentist is part of the plan please call 800-290-0523.

Vision is through Avesis and vision providers can be found on the vision provider page link on page 2 or at www.avesis.com .  To get your eye exam and glasses with the co-pays you must use a provider in the network.
