ATTENTION SLMPD RETIREES

 

If you were under 65 years of age anytime between September 2006 and November 2008 you are included in the settlement of the Retiree Health Insurance Class Action Lawsuit filed against the SLMPD.

You should have received a notice in the mail from the Department.  To get your share of the settlement you MUST complete the form and mail it to the Settlement Administrator.

Deadline for returning the form is June 14, 2010.

If you return your form by June 1, 2010, you can check a list that will be posted on the Pension website on June 2 to confirm that your form has been received.

 

Confirm that your claim has been received by the Administrator by finding your name in the Settlement List put on-line on June 2.
